The personal protective equipment (PPE) template is used to record the issue and check of personal protective equipment. The template records the acknowledgment that instructions for the correct use of PPE has been given, provides space to document regular checks of the PPE as well as outlining what to check for with each item of PPE. PPE included in the form: ear muffs, eye protection, hard hat, safety footwear, and respiratory protection. PPE must be checked regularly to ensure it is still in an acceptable condition to use.
