Health and Safety Responsibilities

This form is used to allocate health and safety responsibilities.

The responsibilities are to be reviewed annually and may form part of the annual health and safety review meeting, or reviewed separately.

It has a list of responsibilities and space to allocate them. The list covers all different areas but it is not exhaustive, there may be other responsibilities you may wish to add.
